North Range Behavioral Health - Where Hope Begins and Change is Possible
North Range Behavioral Health, Evans Police Department, and Greeley Police Department have partnered together to create the GEM (Greeley Evans Mobile) team. The project will align and embed behavioral health staff to each respective police department, allowing for a behavioral health specialist to respond on scene in the community with public safety officers. Two full time peer specialists will be aligned with the team to provide follow-up, outreach, and care coordination of those who have received GEM services to increase the connection and engagement with the broader behavioral health supports in the community.
Provide a supportive environment to help clients identify their own goals toward recovery and/or resiliency. Teach skills to work toward the client's self-identified life goals (social, educational, housing, benefits, illness management, etc.). Use unique personal experience to engage clients in a peer-to-peer capacity. Provide on-going appraisal of client and family situation. Secure resources and services for clients with mild to severe mental illness, disorders and disabilities. Coordinate with internal as well as external supportive resources to deliver needed services to clients. Collaborate with treatment team members regarding services in response to treatment plan.
- High School Diploma and/or ability to successfully demonstrate reading, writing, and speaking skills, or equivalent combination of education and experience.
- Successful completion of Peer Specialist training that meets core competency standards as specified by the agency is highly desired
- Bilingual/Spanish preferred
- Valid Colorado driver's license and driving record that meets with hiring requirements as set forth for someone that must transport customers/program participants
- Must have personal experience by having been impacted by mental illness, substance abuse issues, child and family issues and/or other co-occurring challenges; have association with the recovery/peer support movement, or been in recovery, personally, for at least one year
- Must have sensitivity to individual needs and show respect for a person's independence and ability.
- Motivated to support others in the recovery process.
- Participation in a Clubhouse, drop-in center, peer run psycho-social rehabilitation program, or other peer and/or family support setting preferred
- Knowledge of motivational interviewing preferred
- Ability to take additional peer-to-peer training as required by the job
Knowledge, Skills, and Abilities:
- Mild to severe mental illness, substance abuse, loss/grief, other disorders and/or disabilities and the recovery/resiliency process.

- Provide services for clients as assigned by Administrative/Clinical Supervisor.
- Use personal experience to model the value of every client's experience and to teach effective coping techniques and self-help strategies that lead to recovery and/or resiliency. Offer clients hope regarding recovery/resiliency, through appropriate self-disclosure, role modeling and coaching during the recovery process and/or development of resiliency.
- Collaborate with therapists to coordinate services in response to the client needs and behaviors to achieve goals of service/treatment plan as assigned; prevent deterioration, and or hospitalization. May be called upon for appraisal of client needs to make appropriate recommendations for services.
- Coordinate a wide variety of services for clients to receive benefits and access resources from internal and external service providers. May help identify client's needs and provide access to services.
- Assist clients to identify barriers to recovery/resiliency and individual warning signs; help cultivate positive behaviors to prevent relapse, avoid future crisis involvement, and focus on wellness.
- Assist clients in improving their well-being and quality of life by coaching in behavioral skills; educate clients to make informed choices; help the client problem solve and negotiate tasks/activities of daily living. May provide a listening ear to clients when needed.
- Teach and role model the importance of medication monitoring, clear communication with medical and mental health providers as needed; may assist client to develop a wellness or recovery/resiliency plan (i.e. SAMHSA, Psychiatric Advance Directive, crisis plan, etc.).
- Provide support and encouragement to the client and/or their family members.
- Advocate for the client in many different forums including, but not limited to, the school system, nursing homes, and with family.
- May lead and facilitate social/recreational activities, support groups and/or consumer-run groups.
- As required by program, document progress reports, mentoring notes, letters, faxes, and phone calls; maintain electronic health records in accordance with agency standards; confirm coding of services rendered is ethically appropriate and results in successful billing.
- Assist with client understanding of service charges and fee changes when applicable; demonstrate fiscal responsibility by supporting agency collection policies and procedures as necessary.
- Work with clinical team to request appropriate authorizations prior to provision of services to ensure payment of services as required by program.
- Establish and maintain close and cooperative relationships with community agencies.
- May be called upon to translate in situations where client is mono-lingual.
- May transport clients to and from various appointments per program requirements, usually with another staff member.
- Perform other duties as assigned.
Work is generally performed in a standard office environment. Work off-site and in the community, including homes, community agency sites, hospital emergency rooms, psychiatric units, nursing homes, jails and juvenile detention facilities. Regularly works with people in acute distress or crisis. May require travel to several sites throughout Weld County to attend meetings.
Dependent upon skills and qualifications. Salary placements generally occur near the minimum to midpoint of the range.
