The base compensation represents the entry point of the range. Actual salaries will vary and may be above or below the starting wage based on factors including but not limited to location, experience, education, and licensure. Salary placements generally occur between the starting wage and the midpoint of the salary range.

Position Summary:
With a Customer First and Compassionate approach, Case Managers will:

  • Provide services to clients as assigned by the Administrative/Clinical Supervisor. May carry a caseload assigned by the Administrative Supervisor.
  • Assist clients in achieving their goals as stated in their treatment plans or as indicated by appropriate brief interventions. 
  • Ensure service engagement and delivery as necessary by adapting to client needs, visiting clients in various environments, conducting active outreach to clients, accompanying clients to appointments, providing childcare when needed to assist the client, and setting up medications in med-minders.
  • Monitor, evaluate, and record client progress concerning treatment goals. Use electronic systems to document progress reports, case management notes, letters, and phone calls. May input, organize and maintain detailed clinical performance data to comply with applicable government or program requirements. 
  • Collaborate with therapists and other relevant staff to coordinate services and address client needs and behaviors; percent deterioration and/or hospitalization through consistent and adequate monitoring as indicated by program scope.

 Education Requirements:

  • Bachelor's Degree in Human Services from an accredited college or university required. 

Minimum Qualifications:

  • 1-year experience working with individuals with substance use disorder challenges or mental health challenges is preferred.
  • Experience performing case management and/or advocacy preferred.

Nice to Have: Monetary Incentive Available 

  • Bilingual Spanish speaking preferred
