The base compensation represents the entry point of the salary range. Salary placements will vary and may be above the starting wage based on factors including but not limited to location, experience, education, and licensure. Salary Placements generally occur between the start wage and the midpoint.

Program:
North Range Behavioral Health is the recipient of federal funding through the Substance Abuse and Mental Health Services Administration (SAMHSA) to become a Certified Behavioral Health Clinic (CCBHC). North Range Behavioral Health is now one of four centers in Colorado to join others across the nation. The CCBHC, or Access Team, is now an innovative multidisciplinary team initiating immediate access to behavioral health care. The team includes clinicians, case managers, navigators, and peers working together to create a seamless and smooth front door for our large community behavioral health center in Weld County, CO.

Position Summary:
With a Customer First and Compassionate approach, Case Managers will;

  • Embrace the North Range Behavioral Health Mission Statement - Provide compassionate, comprehensive care for people who face mental health and addiction challenges.
  • Collaborate with therapists and other relevant staff to coordinate services, and address client needs and behaviors; prevent deterioration and/or hospitalization through consistent and adequate monitoring as indicated by program scope.
  • Refer, coordinate, and assist clients to access a wide verity of services (e.g., vocational, medical, social security, housing, etc.)  based on need and client desire; aide clients in application processes and provide supporting documentation as needed.
  • Offer support, assistance, consultation, and education to relevant providers, family members/caregivers, and other parties when necessary to adequately support clients and minimize barriers.
  • Assist clients in improving their well-being and quality of life by coaching in behavioral skills; educate clients to make informed choices; develop coping skills of client; help client to problem solve and negotiate tasks of living daily.

Education Requirements:

  • Bachelor's Degree in Human Services area from an accredited College or University.

Experience:

  • No experience required

Minimum Qualifications:

  • Valid Driver's License and driving record that meets agency insurability requirements.

Nice to Have, Monetary Incentives Available:
